What Should I Bring?

  • Photo ID
  • Medi-Cal, Medicare card or private insurance card
  • Immunization records
  • All medication containers

Can I Schedule a Same Day Appointment or Just Walk In?

A limited number of same day appointments are available each day, please call our office for availability. We recommend that you call or come in to the health center to schedule an appointment; we will make our best efforts to accommodate walk-in appointments.

How Can I Pay for My Visit?

Livingston Community Health accepts Medi-Cal, MediCare, Covered California plans, most insurance programs, and self-payments. A sliding fee discount is available for self-pay patients based on family size and income. We also offer free application assistance if you need help paying for your visit. Just bring us a government issued photo ID and paystubs or tax return.
